Chapter 7 Terminal Discharge Pathway
Process & Workflow of Terminal Discharge
Discharge Planning and Management
Patients’ preferences for care planning should be explored as early as
possible even while they are well so that they have time to organise and
settle their unfinished business. It should include topics such as surrogate
decision maker, preferred caregiver, preferred place of care, preferred
place of death and will writing. If the preferred place is home which may
require terminal discharge, the clinician should also conduct discussions
with the family and/or caregivers to explore this option.
Prior to terminal discharge, the primary team that is caring for the patient
should ensure that:
1. Family and/or caregiver has completed the caregiver training (CGT)
and is competent and confident to take care of the patient at home.
2. The primary team should facilitate and support the family and/or
caregiver to obtain the necessary equipment at home such as
oxygen concentrator or hospital bed.
3. Family and/ or caregiver to arrange homecare (family or private
nurses or caregivers) and transport home (e.g., private ambulance).
4. Adequate medications are prepared according to the assessment
and prescription from the doctor. If subcutaneous medication is
prescribed, then to insert a subcutaneous port prior to discharge as
illustrated.
5. To give information to family and/or caregiver as per 7.1 terminal
discharge checklist.
6. To prepare documents according to terminal discharge toolkit (7.1
to 7.10).
