Terminal Discharge Pathway
Workflow and Descriptions for Medication
Important workflow Descriptions
1 Preparation of i. Doctor (Consultant, Specialist or Medical
prescription Officer) to prescribe medication, maximum 5
to 10 doses of each subcutaneous
breakthrough crisis medication, not
exceeding 14 days.
2 Supply and collection i. Inpatient pharmacy to supply medication
from Pharmacy ii. Ward nurses to collect medication from
inpatient pharmacy
3 Preparation of i. Staff nurse to prepare subcutaneous
Injectables medication (single dose per syringe) in ward
according to label (name, dose, dilution, and
route).
ii. Ward pharmacist to countercheck
medication to ensure accurate dose, dilution,
and preparation.
4 Education for i. Staff nurse and/ or pharmacist to educate
caregivers regarding medication including indication,
route of administration, interval, refill,
storage, disposal and recording in the
medication diary.
5 Record of use i. Caregivers to record date, time and
indication of medication administration
in the medication diary.
6 Refill Medication i. Only pre-appointed person can collect refill
medication after reassessment by the doctor.
They will need to fill in their name, NRIC
number, relationship to patient during the
collection.
7 Return and disposal of i. All used and unused medication syringes
medication must be returned to the Hospital Kuala
Lumpur Pharmacy for proper disposal.
ii. Upon returning used and unused medication,
the pharmacist will calculate and document
the number of returned syringes.
