Chapter 5 Manpower Requirement
Description Role And Responsibility
1 Primary or Palliative i. Discuss with patient, family and caregiver,
Care Doctor as early as possible, regarding goals of
care including aim of care (e.g. active,
comfort care or symptom control) and
preferences for discharge destination (e.g.
home, hospital or nursing home). The
discussion should include preference for
future hospital admission. This
conversation is important especially if the
prognosis is short e.g., hours to days.
ii. Educate family and caregiver regarding
caregiver training, home preparation,
prognosis and symptom management.
iii. Prescribe medication and to complete
documentation as per Toolkit in Chapter 8.
2 Pharmacist i. Review discharge prescription in
accordance with patient’s clinical notes.
ii. Responsible for recording and supplying
medication including sublingual or
subcutaneous medication.
iii. Counsel and dispense medication to
patient, family or caregiver.
3 Nursing team i. Conduct Caregiver Training, CGT to family
or caregiver which includes nursing care,
repositioning for respiratory secretions,
stoma care, handling of tubes (e.g.
nasogastric tube) and management of
wounds.
ii. Guide patient, family and caregiver
regarding equipment and transport back
home.
