Terminal Discharge Pathway
Patient Selection (Criteria)
Important considerations for terminal discharge are the inclusion and
exclusion criteria. This section also includes the role of Palliative Care
Team in Terminal Discharge and on recognising signs of the patient whom
dying is imminent.
Inclusion Criteria
• Patients who are assessed and deemed imminently dying
(prognosis being hours to days) by primary team
• Primary team have counselled patient/caregivers regarding
diagnosis/prognosis
• Patients and caregivers who explicitly state their preference/wish to
die at home
• Caregivers are willing and able to provide end-of-life care for patient.
Exclusion Criteria
• Patient and caregivers who have difference of opinions on preferred
place of death
• Patients with indefinite prognosis and with possibility of recovery.
• Caregivers that are unwilling or hesitate to bring the patient home
for end-of-life care.
Palliative Team Referral Criteria
• Complex physical, psychosocial, or spiritual issues identified in
patient or caregivers.
• Assistance requiring palliative team input.
