This delay in surgery leads to cataract-related visual complications, causes overcrowding,and increases cost to patients and hospital. This study aims to reduce the percentage of repeatedpre-operative assessment visits prior to cataract surgery.KEY MEASURES FOR IMPROVEMENT:The key indicator for improvement was measured using the percentage of patients that came for repeatedpreoperative cataract assessment. The standard is <30% based on department consensus approved by thehead of department.PROCESS OF GATHERING INFORMATION:This is a quality improvement study that was conducted using universal sampling technique in threeconsecutive cycles from January 2022 to November 2023. Data was collected using a manual data formand notes were traced using e-hospital information system version 12 (eHIS12x).ANALYSIS AND INTERPRETATION:Pre-remedial study showed 92% of patients had repeated preoperative assessment visits prior to cataractsurgery. The contributing factors were delay in intraocular lens aid application (31%), non-optimisedmedical (24%) or cardiology issues (23%) and sudden changes in type of anaesthesia (14%).STRATEGIES FOR CHANGE:The strategies include the change of process of care, checklists for staff and patients, new guidelines fordoctors, cataract booklet and conducting cardiac risk assessment and ECG course seminars to increaseconfidence in treating patients with co-existing cardiac issues.EFFECT OF CHANGE:The percentage of repeated preoperative assessment significantly reduced from 92% to 67% after Cycle 1,then down to 37% and 31% after Cycle 2 and Cycle 3, respectively. ABNA is reduced from 62% to 1%.THE NEXT STEP:We would like to expand the usage of the guideline created with other Ophthalmology Department indifferent hospitals.
