Objective

1. To provide a standardised structure to select suitable patients and
facilitate the process of terminal discharge.

2. To provide comfort and relief of distressing physical symptoms
related to advanced and incurable progressive life-threatening
conditions at home via sublingual and parenteral (subcutaneous)

routes for patients who are at end of life.

Scope of service

This guideline provides the details regarding management of patients for
terminal discharge, including outlining the details of selection criteria,
responsibilities, algorithms, management of medications and

accountability pathways in the setting of Hospital Kuala Lumpur.

Patient population are those diagnosed with advanced disease including

cancer, organ failure and genetic disorder.
