               RESULTS: There were 20 patients involved in this study with the majority of them were female
               (75%) and more than 65 years old (80%). The average DFIT score on the first visit was 72%
               and average compliance score 76%. HCPS led by the pharmacist was able to increase the
               medication compliance score by 15% whereby medication knowledge increased by 25%.
               DISCUSSION/CONCLUSION: HCPS is a patient-focused service that is potential to empower
               patients on proper management of medications and subsequently improve patients` health.
               Better strategies should be planned and implemented so that more patients would benefit
               from the service.

               INTRODUCTION: Microbial contamination can occur at any stage of food production which
               were from food services and storage or originating from raw materials, food handlers and
               environmental. This study was conducted to assess the microbiological prevalence of Nasi Air
               Dingin (NAD) which were ready-to-eat (RTE) food distributed from Kelantan to Selangor on a
               daily basis.
               METHODS: A total of 52 samples of NAD had been analyzed for six microbiological parameters
               which consists of Aerobic Plate Count (APC), Coliform, Escherichia coli (E. coli), Bacillus cereus
               (B. cereus), Staphylococcus aureus (S. aureus) and Salmonella spp.
               RESULTS: One out of 52 samples were detected with both S. aureus and E. coli namely Nasi
               Gulai Ayam. Meanwhile, 10 out of 52 samples were detected with single pathogenic bacteria
               including Salmonella detected in 2 samples of Nasi Kerabu Ayam, S. aureus detected in 3
               samples of Nasi Dagang Ikan and one samples of Nasi Gulai Ayam, B. cereus detected in 2
               samples of Nasi Kerabu Ayam and 1 sample of Nasi Gulai Ayam and E. coli detected in 1 sample
               of Nasi Kerabu Ayam.
               DISCUSSION/CONCLUSION:  A  possibility  of  health  risk  indicated  by  the  presence  of
               pathogenic  organisms  which  implicates  to  food-borne  diseases.  Food  handlers  are
               recommended  to  obtain  Home-based  or  Trust  MyCatering  Recognition  from  Ministry  of
               Health to ensure compliance to safe hood handling.  Distribution of NAD from Kelantan to
               Selangor in chill temperature could also be considered.

               CASE DESCRIPTION: This case entails a 80-year-old lady with underlying Diabetes Mellitus,
               Hypertension and Osteoporosis, presented with fever and respiratory symptoms associated
               with lower back pain. Initial laboratory tests revealed Haemoglobin, Ferritin and C-Reactive
               Protein level of 7.3 g/dL, 271.2 µg/L and 95.9 mg/L respectively. Full Blood Picture showed
