INTRODUCTION: Despite the public health sector is heavily subsidized by the government,
Malaysia’s out-of-pocket (OOP) expenditure remains high at 35% of its total healthcare
expenditure. Heavy reliance on OOP at the point of care poses a great threat to financial
impoverishment of the household. This study aims to examine the trend of incidence and
inequality of unaffordable healthcare financing among households in Malaysia.
METHODS: This study analysed household data from 3 round of National Health and
Morbidity (NHMS) survey; 2011, 2015 and 2019. Unaffordable healthcare financing was
defined as OOP for healthcare financed through borrowing (with or without interest) or sale
of assets. Descriptive statistics were conducted and economic inequalities in distress
financing were determined using the concentration curve and concentration index. Analysis
               was conducted using STATA 14.
RESULTS: The incidence of unaffordable healthcare financing among household was 13.2% in
2011, 19.7% in 2015 and 14.9% in 2019. The distribution of unaffordable healthcare financing
throughout the years was found to be higher among the poorer households. Concentration
index for unaffordable healthcare financing was -0.134 in 2011, -0.257 in 2015 and -0.246 in
               2019.
DISCUSSION/CONCLUSION: Despite initiatives and policy by the government for financial
protection, incidence of unaffordable healthcare financing remains the same throughout the
years while the economic inequality of unaffordable healthcare financing distribution is
affecting those from the lower socioeconomic group Additional financial protection is
               required especially among this group.
