Insufficientlydocumented ADR reports are excluded from further pharmacovigilance assessments. The verificationstudy (2022) showed that only 34.38% of ADR reports received by the Pharmacy department were ofgood quality.KEY MEASURES FOR IMPROVEMENT:The indicator measured was the percentage of good quality ADR reports. The standard was 100% basedon the requirement set by the National Pharmaceutical Regulatory Agency (NPRA).PROCESS OF GATHERING INFORMATION:A quality improvement study was conducted from June 2022 until December 2023, including all ADRreports except those involving vaccines, diagnostic substances, supplements, over-the-counter (OTC) andtraditional medicines. Questionnaires regarding knowledge and perspective of ADR were given tohealthcare professionals and remedial action design based on data received. Good quality ADR wasmeasured by percentage of quality ADR report based on AQUA-12.ANALYSIS AND INTERPRETATION:The contributing factors included no reporting feedback (25.7%), lack of awareness (21.9%) and lack ofknowledge (19%).STRATEGIES FOR CHANGE:The strategies included attaching a Reporting Guide to the ADR Reporting Form, providing an ADRHotline Number, organising ADR Webinar Series, publishing the ADR Newsletter, conducting ADRAwareness quizzes, continuous education for healthcare professionals and a briefing on ADR reportingduring new staff orientation.EFFECT OF CHANGE:The percentage of good quality reports increased from 34.38% to 71.43% across three cycles. ABNAimproved from 65.62% to 28.57%. Contributing factors such as no reporting feedback (14.9%), lack ofawareness (11.1%) and lack of knowledge (8.2%) also decreased.THE NEXT STEP:Continuous monitoring of ADR reporting is essential, alongside sustaining remedial measures.
