Foreword

“To cure sometimes, to relieve often, to comfort always”

There is never a more fragile time in life when you know the end is
near. Though it is a moment of sadness and upcoming grief, it doesn’t

mean that there is nothing can be done. Having the ability to choose where
the last chapter of life is spent is something that we should treasure.

A lot of planning and thought is poured into carrying out a procedure or
surgery. Similarly which medication to use for which certain condition is
also important. There are times though, it seems like that there is nothing
more that can be done, and the patient is at the end of their life. This is

however quite far away from the truth. During this critical phase, having a
clear plan and honest open communication can clear a lot of doubt and
fears of both family and patient.

Therefore is my great pleasure to see this guideline be finally finish.
Knowing that the team behind it has poured in hours of work to ensure all

aspects are covered.
