dedicated a large amount of their precious time and their prevailing
commitment to this project is deeply appreciated and cherished. I also

acknowledge with much gratefulness towards the reviewers for their
experiences and knowledge input. I would also like to thank Dr. Denesh
Menon Vijayan, Ms. Elaine Kan Mei Ying, Mr. Victor Ong Sheng Teck and

Ms. Adelin Yong Sue Wen for their unwavering support and comments.

In addition, I also cherish and value our team members in Palliative Care

Unit, Hospital Kuala Lumpur: Dr. Mohd Fadzly bin Baharum, Dr. Kohmilah
Alagar, Dr. Rebecca Tan Bee Ee and Dr. Jessamine Chong Sze Lynn for
their continuous hard work and enthusiasm, to overcome challenges and
obstacles to fulfil patient’s desire to be home.
