It is my honour and delight to present our very own Hospital Kuala Lumpur
Terminal Discharge Guideline. This project is made possible with the
combined effort and expertise of multidisciplinary healthcare
professionals since September 2021. We have also strived to adapt to our
local culture and current healthcare settings
We strongly believe that everyone has a right to palliative care and if
circumstances permit, we would like to honour our patients’ preference to
be home as much as possible, especially during their last few days.
Therefore we hope that this guideline will be able to assist healthcare
professionals carry out a smooth and systematic process for discharge
with the aim to help patients achieve their desire to spend their last days
at home, surrounded by their loved ones in peace and comfort.
Firstly, I thank God for the opportunity, wisdom and discernment to
complete this project. He has blessed us with many experienced people
who have been instrumental in the successful completion of this guideline
with their encouragement and valuable expert opinions.
Next with great respect I would like to express my gratitude to Dr. Marina
binti Abdullah Sani, the Deputy Director of Hospital Kuala Lumpur, for her
tremendous support which not only expedite the process but also is a
source of inspiration and motivation for us. I would also like to thank Dr.
Hazirah binti Mohd Nor and Dr Mohamad Nazrin bin Mat Dahala for their
efficient administrative assistance.
