How do we do it?

We collected all the patients’ records from the respective hospitals and we discussed
each case in detail during the mortality review meeting. Bear in mind, we have to look

at every page on the patients’ clinical case notes. We got the committee members to
review and present the cases and the committee comments on the management

course of the case. Subsequently, based on the discussion, the committee came out
with the recommendations based on the cases.
