in category 4, we started them on anticoagulant prophylaxis. However, the current
literature or data from Europe showed that a lot of these patients while on prophylaxis
are still getting venous thromboembolism. There is a recommendation to use high
prophylactic dose anticoagulants for these patients.
What is called a high prophylactic dose anticoagulant? Meaning the dose is
something like enoxaparin 0.5 mg/kg 12 hourly. It is still controversial as different
centres recommend different things. Some recommended high prophylactic dose
anticoagulants for all ICU patients and some said we should base on the increased D-
dimers values. Again, the D-dimers cut off values are different. Some use the value of
3000 as cut-off and some use the value of 5000 as cut–off point. We will discuss this
and come up with a consensus.
