                                    Q Bulletin, Volume 1, No. 32 (Supplement 1), Jan - Dec 202412th National QA Convention, 8 %u2013 10 October 2024OP-05Towards Reducing the High Percentage of Iron Deficiency Anemia in PregnantMothers in Klinik Kesihatan Sungai AchehKhaw CK1, Nithiyia R1, Asilah AR1, Mastura MI1, Norlia J1, Mohamad Hafizan I1, Thanaeswari BK1,Farah R1, Izzati I21Klinik Kesihatan Sungai Acheh, Pejabat Kesihatan Daerah Seberang Perai Selatan, Penang2Unit Permakanan, Pejabat Kesihatan Daerah Seberang Perai Selatan, PenangSELECTION OF OPPORTUNITIES FOR IMPROVEMENT:Klinik Kesihatan Sungai Acheh (KKSA) exhibited the highest prevalence of iron deficiency anemia (IDA)in pregnancy in Penang State in 2021 at 14.7%, leading to adverse outcomes, including blood transfusionsand stillbirths.KEY MEASURES FOR IMPROVEMENT:The indicator was the percentage of pregnant mothers with IDA at 36 weeks period of amennorhea, with astandard of %u22644%. IDA is measured as Haemoglobin less than 11g/dL with serum ferritin < 30 mg/nL.PROCESS OF GATHERING INFORMATION:A quality improvement study was conducted from October 2022 until March 2024, consisting of onepre-remedial and three remedial cycles, using the universal sampling method and included all pregnantmothers with minimum two antenatal visits. Sampling tools included patient knowledge questionnaires,counseling checklists, staff knowledge questionnaires, and IDA Quality Audit forms.ANALYSIS AND INTERPRETATION:Pre-remedial IDA percentage stood at 14.5%, with factors including poor haematinic compliance(53.3%), ineffective IDA counseling (36.6%), and inadequate IDA management (23.3%). The ABNA was10.5%. Five mothers required blood transfusion and two cases of IDA-associated stillbirths.STRATEGIES FOR CHANGE:Strategies included improving counseling through innovative digital tools in Malay language, increasinghaematinic compliance with compliance checklist and multidisciplinary activities, and optimising IDAmanagement with in-house IDA management protocol.EFFECTS OF CHANGE:The IDA percentage reduced from 14.5% to 2.9% post cycle 3. The ABNA reduced from 10.5% to below0%, with improvements in MOGC as haematinic compliance increased from 53.3% to 83.3%, counselingeffectiveness improved from 36.6% to 93.3%, and IDA management improved from 23.3% to 80%. Approximately RM 3022 in costsavings were achieved.THE NEXT STEP:This project was expanded to all clinics in Seberang Perai Selatan District.
