A pilot study conducted from 7th until 20th February 2023 showed that three healthclinics in Melaka Tengah District Health Office (PKDMT) had a high percentage of prescription errorswith 19.33% (116 out of 600 prescriptions).KEY MEASURES FOR IMPROVEMENT:The key indicator for improvement is the percentage of prescription errors. The standard was 18.30%based on Quick Guide Prevention Prevention Error, Patient Safety Unit, Medical Care Quality Section.Ministry of Health Malaysia, 2020.PROCESS OF GATHERING INFORMATION:A quality improvement study was conducted from April until December 2023 on non-communicabledisease (NCD) patients in Tengkera Health Clinic, Peringgit Health Clinic and Batu Berendam HealthClinic. A questionnaire was given to prescribers to determine the contributing factors of high prescriptionerrors. Data was collected from prescriptions, PhIS (Pharmacy Information System) and Buku RekodRawatan Pesakit (clinic copy and patient%u2019s copy).ANALYSIS AND INTERPRETATION:Out of 378 prescriptions, 131 had errors (34.66%). The main contributing factors of prescription errorsincluded improper documentation of medical records (50.9%), lack of awareness of safe prescribingpractice (34%) and ineffective communication between prescriber and pharmacist (15.1%).STRATEGIES FOR CHANGE:To avoid improper documentation of medication records, a %u2018PHARMnote%u2019 was attached to the patientrecord to communicate with the prescriber on the next appointment. In order to increase awareness amongprescribers, a continuous medical education (CME) was given. The reminder sticker was placed near theprescribers%u2019 workplace to remind them to update patient records.EFFECT OF CHANGE:After all the changes made during cycle one, prescription error was reduced from 34.66% to 3.97% thusachieving the standard indicator (18.30%).THE NEXT STEP:All remedial actions will be implemented in all health clinics in PKDMT starting June 2024.229 | Page
