Terminal Discharge Pathway
7.5 Medication Information Sheet for Patient & Caregiver
Medication Description
• Used for: Pain or breathlessness
Morphine/
Fentanyl • Subcutaneous onset: 15 minutes
• Frequency: Every 1 hour if symptom does not improve
• Used for: Restlessness, agitation or seizures
Midazolam • Subcutaneous onset: 5 to 10 minutes
• Frequency: Every 1 hour if symptom does not improve
• Used for: Nausea, vomiting, restlessness or
agitation
Haloperidol • Subcutaneous onset: 10 to 15 minutes
• Frequency: Every 4 hours if symptom does not
improve
• Used for: Nausea, vomiting, restlessness or
agitation
Sublingual • Tablet onset: Hours
Olanzapine • Frequency: Every 2 hours if symptom does not
improve,
• Maximum 20mg/day
• Used for: Seizures
• Per Rectal onset: 2 – 10 minutes
Diazepam
• Frequency: Every 12 hours if symptom does not
improve
• Used for: Respiratory secretions or “noisy
breathing”
• Subcutaneous onset: 10 minutes
Hyoscine
• Frequency: Every 4 hours if symptom does not
improve
• Maximum: 120mg/day
