9 Procedure after death
• To make police report at “999” or “112”
• Plan funeral with funeral service provider

• Emphasize that patient can come back to hospital anytime via
Emergency Department

7.7 Medication Storage, Disposal, Return and Refill
7.8 Letter for Terminal Discharge (to fill in 2 copies)
7.9 Medication Refill Record
7.10 Return of Medication
• Keep in patient’s medical records: 7.1, 7.2, 7.8
• Pass to family / caregiver: 7.2 to 7.10
o To staple it into a booklet
o 7.3 to 7.10 to return to hospital staff when returning used and
un-used medication)
